All appointments at our Clinics are charged at £30 and are payable on attendance at each appointment. Invoices will be provided for all appointments and for insurance companies on request. Home visits are charged at £35 to account for the additional time and travel.

At present we can only accept cash or cheques as forms of payment
(cheques made payable to “formotion physiotherapy” please)

Please also note that if appointments are cancelled within 24 hours of your allocated appointment you may be liable for a standard charge of £15.
